Page Three+ Body of Report Consisting of the following sections and descriptions

  1. Executive Summary. Describe very briefly each problem identified and its associated recommendations. Only a couple of sentences for each issue should be included.
  2. Introduction. This brief section describes who did what where.
  3. Background. This section describes the history and the nature of the problem. Notes taken from conversations or interviews should be placed in this section. All information collected from Phase I of the checklist should be listed here.
  4. Scope of Work. Description of work performed as per the contract including a description of the physical area of focus and limitations of assessment.
  5. Site Visit Findings. Information gathered from the walk through inspection should be summarized here. Any stains, growth, odors or problems should be listed in this section. All information collected from Phase II of the checklist should be listed here.
  6. Measurements. Indicator measurements including temperature, relative humidity, carbon dioxide, pressurization and moisture meter readings should be listed in this section. The table created from Phase III in the checklist should be listed here.
  7. Sampling. Any samples taken in accordance with Phase IV on the checklist should be described in this section. Be sure to include a justification of why sampling was performed. Include information on the laboratory performing the analysis. Include any lab reports as an appendix.
  8. Conclusions. Here you should summarize the problems identified in the assessment and the suspected concerns.
  9. Recommendations. Here you should describe specific implementation steps for the client to follow to resolve issue. Site any standards or guidelines in the justification of your recommendations
  10. Disclaimers. Describe the limited nature of the assessment and the daily/hourly variations of indoor conditions. Outline any limitations you would like to place on the use of your report or responsibility of report. 3rd party reliance used only at that party's risk.
